Join us to learn about the benefits of team-based patient care.

PrimeHealth+ provides primary medical, dental, behavioral health, and pharmacy services to patients of all ages, regardless of income or insurance status.

Join us as PrimeHealth+ team members share information about the PH+ model of team-based patient care, highlight several service lines, and discuss financial assistance qualifications for patients. The team will also provide information about who is eligible to become a patient at PrimeHealth+.

Lunch will be provided.

Caregiver Support Group is typically held monthly on the first Thursday of every month.

Memory Café is a welcoming place for individuals with Alzheimer’s or other types of dementia or brain disorders. Additionally, it is helpful for those with all forms of mild cognitive impairment.

A safe and comfortable space, Memory Café provides an opportunity for caregivers and their loved ones to enjoy delicious food, socialize, listen to music, provide mutual support and exchange information.
